The mandatory fields* must all be filled in otherwise the form cannot be sent.
For the date fields you can overwrite the current year selection and then select month and day.

Please prepare the photos or scans of your ID / foreigner's passport / AHV passport in the format ( .jpg, .jpeg, .gif, .png, .pdf ) in such a way that 3 MB per image is not exceeded.
